Assessment of the Wilson Sex Fantasy Questionnaire among child molesters and nonsexual forensic offenders

Abstract:  "The Wilson Sex Fantasy Questionnaire (WSFQ; Wilson, 1978) is a 40-item self-report questionnaire that assesses 4 types of sexual fantasies: Exploratory, Intimate, Impersonal, and Sadomasochistic. The goal of the present study was to examine the differences between child molesters (n = 64) and nonsexual offenders (n = 41) on the WSFQ. Comparisons included the four underlying factors, 2 factors associated with the fantasizer's role in the fantasy (active vs. passive), and 2 items most closely related to sexual molestation behavior. Results found that molesters reported higher scores on the Exploratory and Intimate subscales, as well as overall fantasy. Scores on the Impersonal and Sadomasochistic subscales were not significantly different. Molesters also reported higher scores on fantasies where they were the actor, and higher scores on fantasies most closely related to sexual molestation behavior. Subsequent analyses found that both offender groups reported significantly lower levels of fantasies than college comparison subjects and noncriminal sexual deviants. Differences among the 2 study groups are discussed in terms of social and cognitive characteristics of molesters."

The lethal paraphiliac syndrome: accidental autoerotic deaths in four women and a review of the literature

Abstract:  "Four previously unpublished cases of female asphyxiophilia are presented. All women were found immobilised by obviously self-tied ropes, string or handcuffs. The women, who were alone at the time of death, died of a lethal paraphilia. The autopsies revealed asphyxiation as the cause of death, caused in two cases by suffocation as a result of hanging and strangulation and in the other two cases by plastic bags placed over the individuals head. In one case there was additional evidence at the scene that the deceased had inhaled ether. In none of the four cases was there any indication that the asphyxiation was due to homicide or suicide. Thus they can be described as accidental autoerotic deaths (AAD). The four cases closely mirror findings from scenes of male AADs, although autoerotic practices are generally believed to be rarer among females than in males."

Abstract:  "It is argued that a new and dramatic increase in acts of sexual perversion (SP) has been seen in recent years, and that concepts and classifications introduced as a result of past research no longer correspond to the current situation. They tend to be too normative, too descriptive, and show a lack of precision. The present author therefore suggests that these concepts be put aside, and that the theories of Freud and Lacan regarding the partial impulse are better suited to this context: SP is in fact a perversion of the partial impulse, and of its 4 underlying constructs: the upsurge, the source, the object and the aim. These factors are utilized by the perverse individual to put into effect the 4 forms of sexuality available to him or her. This approach assumes very different proportions, depending on whether the emphasis is put on one form rather than another, and on whether it relates to fantasy or to reality. On this basis, another classification can be determined which takes into account the entire range of SPs, and which better delineates the distinctions between those SPs exclusively connected with the subject himself, those which require therapeutic treatment, and finally those which require penal sanctions in proportion to the severity of the crime committed."

Autoerotic accident associated with self-applied ketamine

Abstract:  "We present a rare case of an autoerotic accident involving a fatal combination of asphyxia by suffocation and intoxication with self-administered intravenous ketamine. Of note in this case is the fact that the victim was an emergency medical technician. Ketamine causes complete analgesia with superficial unconciousness and amnesia called 'dissociative anasthesia'. Futhermore low anaesthetic doses of ketamine induce alterations in mood, cognition and body image and the substance is an emerging drug of abuse. We discuss the death scene investigation, findings at autopsy and the toxicological report."

Hippocampal pathology in two mentally ill paraphiliacs

Abstract:  "Paraphilias or disorders of sexual behavior have markedly increased in prevalence during the last decade. Until now no published neuropathological studies on paraphilia have appeared in the medical literature. A computerized search was done on all available medical and autopsy records of a large urban hospital (St. Elizabeths Hospital, Washington, DC) for any mention of deviant sexual behavior. Cases were then reviewed for presence of a history consistent with DSM-IV diagnoses of paraphilia. Two such cases were identified. Neuropathological examination in both cases revealed simple cell atrophy of pyramidal cells confined to different hippocampal subfields. Reactive astrocytosis was present in the outer strata of the affected regions. The pathological changes in the hippocampus resemble those reported after persistent stress or long-term chronic glucocorticoid administration. The accompanying astrocytosis indicates a reactive, ongoing process. The findings suggest new therapeutic interventions in the treatment of paraphilia."

Abstract:  "Examines James Joyce's novel Ulysses in terms of fetishism and masochism, both of which are revealed in Leopold Bloom, one of two principal characters in the work. Bloom's example suggests important comparisons with psychoanalytic theories that focus on disavowal of castration as the mechanism both of fetishism and masochism. However, Joyce's delineation of Bloom's consciousness also elaborates a specific link between fetishism and masochism which has not been remarked in psychoanalytic literature. This link in Bloom's thoughts provides specific insights into male fantasies that neither psychoanalytic theory nor Joyce studies thus far have discussed as such. The author considers the 'Circe' episode of Ulysses by focusing on a passage that she believes is problematic, considering the account of fetishism and masochism suggested by the earlier chapters and by the work of the psychoanalysts. This passage is Bloom's elaborate fantasy about being transformed into a womanly figure who is whipped by Bella/Bello Cohen, a prostitute who transforms into a man in the service of Bloom's masochism." Abstract:  "Describes the career of J. G. Kiernan (1852-1923), an American psychiatrist of a new school of thought that championed "scientific" psychiatry, particularly stressing hereditary and biological arguments and agitating against the old guard in psychiatry which was dominated by those who ran asylums. This paper considers the relationship between the emerging discipline of sexology and forensic psychiatry; Kiernan's work spans these 2 disciplines. (Sexology after the 1870s was increasingly interested in explaining aberrant sexual behaviors in terms of biological or congenital factors.) This paper notes Kiernan's work on criminal insanity and criminal responsibility, and his work on 'sexual perversion,' notably homosexuality, atavism, algophily (sadism), lust murder, autoflagellation, and fetishes."

Abstract:  "Humans show a wide array of sexual preferences and behaviors. Although most humans prefer and have sex with consenting adults of the opposite sex, some individuals have unconventional preferences with regard to the sex or age of sexual partners, or with regard to the nature of sexual activities. In this paper, we describe a rare case of preferential bestiality, or zoophilia. The client meets the most stringent criteria for the diagnosis of zoophilia. In particular, his phallometrically measured arousal pattern shows a sexual preference for horses over other species, including humans."

Characteristics of juvenile offenders admitting to sexual activity with nonhuman animals

Abstract:  "Compared the family characteristics, victimization histories, and number of perpetration offenses of juvenile offenders who admitted to having had sex with animals to juvenile offenders who did not. Ss were 381 institutionalized, adjudicated, male youth offenders (mean age 16.9 yrs). Ss completed an anonymous self-report questionnaire. Results revealed that 96% of the juveniles who had engaged in sex with animals also admitted to sex offenses against humans and reported more offenses against humans than other sex offenders their same age and race. Those juveniles who had engaged in sex with animals were similar to other sex offenders in that they also came from families with less affirming and more incendiary communication, lower attachment, less adaptability, and less positive environments. Those juveniles who had engaged in sex with animals reported victimization histories with more emotional abuse and neglect and a higher number of victimization events than other offenders. This suggests that sex with animals may be an important indicator of potential or co-occurring sex offenses against humans and may be a sign of severe family dysfunction and abuse that should be addressed in the arenas of psychological intervention, juvenile justice programs, and public policy."

Acquired sexual paraphilia in patients with multiple sclerosis

Abstract:  "BACKGROUND: Sexual dysfunction in patients with multiple sclerosis is typically characterized by diminished libido, erectile and ejaculatory dysfunction in men, and poor lubrication and anorgasmy in women. In contrast, hypersexual behavior and paraphilias are distinctly uncommon in this population of patients, but have been associated with various focal brain lesions. PATIENT AND METHODS: We describe a man with clinically definite multiple sclerosis who developed profound and abrupt disinhibition and paraphilic behavior during an exacerbation. RESULTS: Neuroimaging revealed a marked increase in the number of enhancing lesions in the right sides of the hypothalamus and mesencephalon and extending into the right sides of the red nucleus, substantia nigra, and internal capsule. The altered sexual behavior was characterized by an obsessive and insatiable desire to touch women's breasts. CONCLUSIONS: Acquired sexual paraphilic behavior is uncommon in patients with multiple sclerosis but may occur when inflammatory demyelination involves the hypothalamic and septal regions of the basal prosencephalon. Our experience with this man illustrates the great difficulty involved in treating such patients when the paraphilic behavior becomes persistent."

The lost laugh: Comedy and perversion

Abstract:  "This dissertation argues that comedy is perverse. All modes of comedy, from puns to feature length films, emerge from the polymorphous perversity of childhood sexuality. This fluidity of early human identity profoundly influences the development of the 'sense of humor' in terms of both form and content. Each chapter combines clinical and theoretical literature on perversion with film and literary scholarship on comedy in order to explicate a specific area of significant overlap between comedy and perversion. By examining these junctions, this project establishes that both phenomena are best understood as psychic strategies that are designed to alleviate anxiety, generate pleasure, and stabilize an identity in crisis. Laughter, typically considered a necessary marker of comedy, signifies the success of simultaneous transgression and restitution. Laughter relies on the psychic defense mechanism of disavowal, the ability to allow co-existence of two contradictory thoughts or images without either one impacting the other. This defensive process is likewise the cornerstone of perversion. Both comedy and perversion are narratives that deny death. Comedy performs this denial through the 'happy ending.' Even the joke has a punchline, which functions as a kind of happy ending. Perversion is a scripted performance that foregrounds risk and danger, but also guarantees the successful refusal of death with the achievement of maximum, possibly orgasmic, pleasure. Both strategies have developed congruent figures that blur or exaggerate signs of sex, gender, and sexuality in order to accept and deny sexual difference. These figures include the absent mother, the phallic woman, the heterosexual couple, the 'inadequate' or gay man, and the male cross-dresser. Perversion and comedy also disavow signs of age and generational difference. By enacting fantasies about age reversals and becoming another size, these strategies fulfill the wish to be younger or older, to be smaller or bigger. The conclusion addresses the seemingly contradictory transgressive, yet conservative nature of both comedy and perversion. The final statements argue that adults can never again play as children do, but that comedy and perversion are ways to resurrect the lost laughter of childhood."

Abstract:  "Sexual obsessions are common symptoms of obsessive-compulsive disorder (OCD). The literal content of these obsessions superficially resembles other types of iterative sexual ideation, as seen in the paraphilias, PTSD, and normal sexual fantasy. However, their form, function, and effect on behavior vary greatly. A failure to distinguish these different categories of sexual thought can lead to confusion and iatrogenic treatment. This paper will describe the content and form of sexual obsessions, clarify salient differences between sexual obsessions and other repetitive sexual thoughts, and finally discuss ways of treating sexual obsessions when they are symptomatic of OCD."

Abstract:  "In the introduction to DSM-IV-TR, the following unequivocal statement is made concerning its changes from DSM-IV: "As with the original DSM-IV, all changes proposed for the text had to be supported by empirical data. Furthermore, all proposed changes were limited to the text sections (e.g., Associated Features and Disorders, Prevalence). No substantive changes in the criteria sets were considered." This claim is misleading. In fact, DSM-IV-TR has made a substantive change to criterion B for paraphilias. The only way the reader would know of this change would be to read all of Appendix D to discover that a change had been made in the paraphilia criteria set. The editors of DSM-IV-TR apparently recognized the unintended and clinically absurd consequence of the change in DSM-IV from DSM-III-R criterion B. According to DSM-IV, the diagnosis of pedophilia cannot be made if the individual acts on pedophilic urges but is not distressed by the urges and is not socially or occupationally impaired. In DSM-III-R, however, acting on the urges alone meets criterion B, as is the case again in DSM-IV-TR for pedophilia and all of the other paraphilias that involve a nonconsenting person. This welcome change in the DSM-IV-TR criteria is certainly substantive."

A DSM-IV Axis I comorbidity study of males (n = 120) with paraphilias and paraphilia-related disorders

Abstract:  "One hundred and twenty consecutively evaluated outpatient males with paraphilias (PAs; n = 88, including 60 sex offenders) and paraphilia-related disorders (PRDs; n = 32) were systematically assessed for certain developmental variables and DSM-IV-defined Axis I comorbidity. In comparison with the PRDs, the PA group was statistically significantly more likely to self-report a higher incidence of physical (but not sexual) abuse, fewer years of completed education, a higher prevalence of school-associated learning and behavioral problems, more psychiatric/substance abuse hospitalizations, and increased employment-related disability as well as more lifetime contact with the criminal justice system. In both groups, the most prevalent Axis I disorders were mood disorders (71.6%), especially early onset dysthymic disorder (55%) and major depression (39%). Anxiety disorders (38.3%), especially social phobia (21.6%), and psychoactive substance abuse (40.8%), especially alcohol abuse (30%), were reported as well. Cocaine abuse was statistically significantly associated with PA males (p = .03). There was a statistically significant correlation between the lifetime prevalence of Axis I nonsexual diagnoses and hypersexual diagnoses (PAs and PRDs). The prevalence of retrospectively diagnosed attention deficit hyperactivity disorder (ADHD) was 35.8%, the third most prevalent Axis I disorder. ADHD (p = .01), especially ADHD-combined subtype (p = .009), was statistically significantly associated with PA status. ADHD was statistically significantly associated with conduct disorder, and both of these Axis I disorders were associated with the propensity for multiple PAs and a higher likelihood of incarceration. When the diagnosis of ADHD was controlled, the differences reported above between PAs and PRDs either became statistically nonsignificant or remained as only statistical trends. Thus, ADHD and its associated developmental sequellae and Axis I comorbidities was the single most common nonsexual Axis I diagnosis that statistically significantly distinguished males with socially deviant sexual arousal (PAs) from a nonparaphilic hypersexual comparison group (PRDs). Sex offender paraphiliacs were more likely to be diagnosed with conduct disorder, alcohol abuse, cocaine abuse, and generalized anxiety disorder. The prevalence of any ADHD in the sex offender paraphiliacs was 43.3%, and nearly 25% of offenders were diagnosed with ADHD-combined subtype."

Abstract:  "In this article, the second of a two-part series, the authors present information on the clinical assessment of individuals with paraphilias and hypersexual disorders. They review ethical considerations in the assessment and treatment of individuals with paraphilias. The role of interview and subjective and objective instruments in the assessment of individuals with paraphilias and hypersexual disorders is discussed. The authors discuss the use of penile plethysmography or phallometry, polygraphy, and viewing time assessments. The authors then discuss behavioral, environmental, and psychopharmacological treatments for paraphilias and hypersexual disorders. The authors describe cognitive-behavioral techniques for decreasing and/or controlling sexual urges (e.g., satiation, covert sensitization, fading, cognitive restructuring, victim empathy therapy) as well as methods for enhancing appropriate sexual interest and arousal (e.g., social skills training, assertiveness skills training, sex education, couples therapy). The article concludes with a treatment algorithm, which suggests beginning with less restrictive treatments (e.g., behavioral or verbal therapies), if possible, and moving to more restrictive alternatives (e.g., biological therapies, institutionalization) as needed."

Anorexia, masochism, self-mutilation, and autoerotism: the spider mother

Abstract:  "In summary, both self-mutilators and eating-disordered individuals come from dysfunctional homes with a very controlling mother and usually absent father. They often have a history of trauma. They are depressed and obsessive, attached to their mothers, who discourage attempts at emancipation. The symptoms serve the purpose of keeping them as little girls with negative feelings toward menstruation, sexual maturity, development, and femininity in general. These symptoms comprise self-destructive behavior in the service of removing sexual thoughts, temptation, and activities. Favazza (1987) included both eating disorders and self mutilation in his 'deliberate self-harm syndrome.' The symptoms, whether they be anorexic, bulimic, or a form of self-mutilation are seen as 'autoerotic in nature and a substitute for normal masturbation' (Hull & Lane, 1988). Eating disorders and delicate self-mutilation are said to have 'a cathartic, self-purifying, function in that they modulate states of anxiety, sexual tension, anger or dissociated emptiness, and they bring about a tremendous quasi-physical sense of relief' (Cross, 1993, p. 50). These patients' use of substitutes prevents maturation and growth as women, causing regression to pregenital phases with the use of pregenital defenses, and the demise of the demands of puberty and mature sexuality."

However, a trend toward treatment outcome studies is emerging. At present, there is consensus that cognitive/behavioral therapies form the standard against which other approaches must be judged. Behavior therapies include various forms of aversive condition, social skills training, biofeedback, and sexual impulse control training, among others. Cognitive approaches include restructuring cognitive distortions, relapse prevention, and empathy training. Somatic treatments have also been reported, including the administration of medroxyprogesterone acetate (MPA) and cyproterone acetate (CPA); both reduce circulating testosterone. Unfortunately, large outcome evaluations of any of these treatments are still lacking."

Abstract:  "Evaluated whether there are any demographic variables or comorbid sexual disorders that distinguish Ss with telephone scatologia (TS) from Ss with other paraphilias (PAs) and paraphilia-related disorders (PRDs). The authors explored the comorbid relationship among PA and PRD groups to assess whether a particular PA or PRD was statistically significantly associated with the TS group. Data were collected prospectively from 206 consecutively evaluated outpatient males (mean aged 15-69 yrs) referred to a psychiatrist. Ss completed an Intake Questionnaire that gathered demographic data. Lifetime sexual diagnoses were assessed using a structured Sexual Inventory and confirmed by psychiatric interview. Findings of greater numbers of associated PAs and PRDs and significant comorbid association with voyeurism, compulsive masturbation, phone sex dependence, and trend association with exhibitionism add to the growing body of empirically based research characterizing TS. Ss in the TS sample did not limit their sexual impulsivity to obscene telephone calling. Rather, they had greater numbers of multiple lifetime deviant and non deviant sexual impulsivity disorders even when compared to other paraphiliacs."

Abstract:  "Discusses reasons underlying the relative absence of research on the leather/levi community in gay and lesbian studies, and the history and special collections of the Leather Archives and Museum (LA & M). At the beginning of the 1970s the leather community of both dominant and submissive individuals had in place a framework of social forms used completely or in part in all US cities where a coherent leather-identified gay community had emerged. However, there is a relative absence of contemporary, primary documentation of this period outside of private records of leather clubs themselves. Oral histories have assumed a greater significance in the leather documentation corpus than is true for mainstream gay and lesbian cultures. From its initial plans in 1992, the LA & M held its 1st public display in 1993, moved to a Chicago storefront location in 1995, and then moved to a permanent location in 1999."

Reversible transvestic fetishism in a man with Parkinson's disease treated with selegiline

Abstract:  "Dopaminergic therapy in patients with Parkinson's disease may change the quality as well as the quantity of sexual interest and behavior. This 72-year-old man had a 37-year history of Parkinson's disease treated with a right thalamotomy and was later treated with levodopa for more than 20 years. Selegiline (5 mg twice daily) was added for motor fluctuations. He developed a frequent impulse to wear women's clothing but did not act on this impulse until his wife died over a year later. He then began to dress in women's clothing an average of once per week. He stated he had never thought of cross-dressing previously. The selegiline was stopped, and his urge to wear women's clothing ceased. Paraphilias are a rare behavioral complication of Parkinson's disease treatment. Other paraphilias have been attributed to dopamine agonists, suggesting that the action of the monoamine oxidase inhibitor responsible for the patient's transvestism in this case was dopamine potentiation. Drug-induced paraphilias and hypersexuality may represent a reversal of the putative premorbid Parkinson's disease personality traits of introversion, cautious behavior, and lack of "novelty-seeking." A biologic basis for transvestism, and paraphilias in general, is not known. Rare clues emerge from cases similar to this one."

Abstract:  "Focused on the ways in which 12 sadomasochistically-oriented males have incorporated, during the last 2 mo, the use of animals into their sexual practices. Subjects (Ss) were derived from a larger survey of the sexual practices and characteristics of sadomasochistically oriented individuals. A matched control groups of 12 Ss was also selected from the initial sample. All Ss completed questionnaires. The bestialic Ss had both become aware of their sadomasochistic interests and started practising sadomasochistic sex relatively late. Ss had seldom engaged in sexual activity with animals in childhood or adolescence. They were likely to think that sadomasochistic sex was good for their psychological well-being, that they were in control of their bodies while having sadomasochistic sex, and that they felt safe when practicing sadomasochistic sex. The bestialic Ss were engaged in a wider range of sexual and sadomasochistic behaviors. It is suggested that sadomasochistically-oriented individuals who have used animals as part of their sexual activity are overall sexually experimental. Also, they do not closely resemble earlier descriptions of individuals who engage in sexual activity with animals."

Abstract:  "This study explores the extent to which 29 individual sexual behaviors, previously identified in L. Alison, P. Santtila, N. K. Sandnabba, and N. Nordling (2001), as each being associated with 1 of 4 different broad themes of sadomasochistically oriented behavior, represent different scales of intensity within each theme. The themes were as follows: hypermasculinity, administration and receipt of pain, physical restriction, and humiliation. For example, we wanted to establish whether skinbranding is a more intense expression of pain than spanking; whether faceslapping is a more intense expression of humilitation than flagellation, and so on. One hundred and eighty-four participants (22 women and 162 men) who were members of two sadomasochistically oriented clubs gave information concerning which of the 29 sexual behaviors they had participated in during the preceding 12 months. A multivariate analysis (POSAC; Shye, 1994) supported the existence of cumulative scales in each of the 4 themes, suggesting that the order in which people engage in different sadomasochistic behaviors is not random and that specific, less intense behaviors generally precede more intense behaviors."

Abstract:  "This paper discusses the treatment of an autistic 18 yr old male with a severe intellectual disability engaged in auto-erotic asphyxiation. It describes how the legal aspects of the design and implementation of the treatment approach were handled. The treatment approach involved a behavioural and psycho-educational programme. A single case study methodology was utilized in order to assess the effectiveness of the intervention. The intervention led to the learning of new adaptive behaviour. Auto-erotic asphyxiation ceased and the individual's masturbatory behaviour changed sufficiently to reduce the threat of mortality. Qualitatively, a significant reduction in the level of stress experienced by the individual's family was reported. It is concluded that behavioural and educational techniques may be effective in the management of such cases. Further research replicating such techniques with similar cases is required prior to the efficacy of this treatment becoming established."

Alexithymia in patients attending a sexual disorders clinic

Abstract:  "Measured alexithymic traits with the Toronto Alexithymia Scale (TAS; R. M. Bagby, G. J. Taylor, and J. D. A. Parker, 1988) in 170 individuals attending a sexual disorders clinic. The authors diagnosed 114 of the subjects with a sexual dysfunction (81 males and 33 women, mean age 43.21 yrs) and 56 with paraphilic disorders (39 men and 17 women, mean age 42.7 yrs). 20.2% of the dysfunctional group and 26.8% of the paraphilic were determined alexithymic when the TAS was measured categorically. When the TAS was considered as a dimensional variable, the subset with paraphilias had significantly higher scores on the TAS only when depressed mood, as measured by the Brief Symptom Inventory (L. R. Derogatis, 1993), was covaried. These data suggest that alexithymia in both sexually dysfunctional and paraphilic individuals is related to depressed mood, although the effect is more pronounced in paraphilics."

Abstract:  "The detailed description of the analysis of a patient serves to illustrate some of the major dynamic issues of character perversions and their treatment. The term refers, according to J. Arlow (1971), to distorted behavior or character taking the place of what had once been a perversion or tendencies toward perversion. As it applies to sexual perversion, the focus is on the denial of a traumatic perception and the fixation on a reassuring detail in the external world which seems to refute that danger, and just as it specifically applies to fetishism, a screen reality is magically being created to counteract the traumatogenic anxiety. Based on the author's experiences, criteria for character perversion are suggested: severe trauma, both physical and emotional; splitting and denial; attempts at magical realization of a countervailing fantasy as wish fulfillment and illusion in the transference; sexualization of the underlying traumatic experiences; dehumanization of relationships in general, of sexuality in particular; doubling of the self into a sadomasochistic and a depersonalized part identity; splitting of the superego; denial of the most important differences; and denial of the flow of time, and of change altogether."
