OCD and transvestism: is there a relationship?

Abstract:  "OBJECTIVE: There have been reports of obsessive-compulsive disorder (OCD) patients with comorbid paraphilias. In this paper, two cases of comorbidity between OCD and transvestism are reported with the aim of discussing possible explanations for this association. Method: Longitudinal case studies of two patients with OCD and tranvestism according to the DSM-IV were assessed using semi-structured interviews. RESULTS: Both patients presented with OCD and comorbid transvestism with different clinical features and treatment response. The case whose cross-dressing was more egodystonic responded better to treatment. CONCLUSION: It is possible that some cases of transvestism are OCD-related while others are more related to a gender identity disorder. A dimensional approach focusing on common phenomenological and neurobiological substrates is suggested as particularly useful for clinical management and future research of both disorders." Abstract:  "There has been increasing interest in the treatment of sexual disorders in recent years. Sexual disorders are classified in DSM-IV as sexual dysfunctions, paraphilias, and gender identity disorders. The sexual dysfunctions are nondeviant or nonparaphillic. The sexual dysfunction disorders should include "hyperactive sexual desire disorder" under sexual desire disorders . Further, there should be a specifier for paraphilias of "with hypersexuality" or "without hypersexuality." There is still incomplete understanding of the neurobiology of sexual disorders although functional neuroanatomy and neoropharmcological research has exposed the neurotransmitters, receptors, and hormones that are involved in sexual desire. Various pharmacological agents including serotonin reuptake inhibitors, antiandrogens, LHRH agonists, and others have been documented as reducing sexual desire. An algorithm for the use of these drugs in the treatment of the paraphilias as well nonparaphilic hypersexuality is outlined. The modes of action, dosages, aims of treatment, and usual methods of prescribing these agents is reviewed in this article. Some future directions of research in pharmacological treatment is also discussed."

The chapter has 4 parts all of which are influenced by events and scholarship in North America, Britain, and Sweden. After an overview of recent developments within ordinary pornography, the author focuses on the disability genre. The author reflects on the ways in which fetishists of disability have assumed prominence in the mainstream. The chapter concludes with a critical exploration of the political implications of these developments."

Abstract:  "Several adult male members of a sadomasochistic sex club were arrested for violating a U.K. statute passed in 1861 - The Offences Against the Person Act. The sex acts spanned a 10-year period and were videotaped. They involved manipulation of the genitalia with hot wax, sand paper, fish hooks, and needles. The sexual interactions were consensual. This analysis tracks the case from trial, to the Court of Appeal, to the House of Lords, and on to the European Court of Human Rights. It examines whether or not these sexual behaviors should be protected under a right to privacy. It contrasts state concerns of bodily harm, albeit consented to, in sex with those incurred in sport."

Abstract:  "Paraphilias (e.g., pedophilia, fetishism) are said to be virtually ineradicable once established. The authors propose that the motivational state known as the Zeigarnik effect according to which interrupted tasks are better recalled than completed tasks, may provide understanding of this process, especially its later addictive-compulsive quality. Reasoning from Zeigarnik-type research, the authors predict a relation between early sexual arousal, its frustration, and subsequent events associated with such arousal. The paraphilias are thus seen as an unusual by-product of a normal adaptive process, that is, a tendency to privilege the recollection of unfinished over finished activities. The authors discuss why paraphilias are associated nearly exclusively with males, and why paraphilic tendencies are apparently quite rare in traditional societies. They also propose new research on the processes and outcomes entailed by the Zeigarnik effect, such research including, but not being limited to, sexuality." (APA/PsycINFO)

Abstract:  "Deaths due to plastic bag suffocation or plastic bag asphyxia are not reported in Malaysia. In the West many suicides by plastic bag asphyxia, particularly in the elderly and those who are chronically and terminally ill, have been reported. Accidental deaths too are not uncommon in the West, both among small children who play with shopping bags and adolescents who are solvent abusers. Another well-known but not so common form of accidental death from plastic bag asphyxia is sexual asphyxia, which is mostly seen among adult males. Homicide by plastic bag asphyxia too is reported in the West and the victims are invariably infants or adults who are frail or terminally ill and who cannot struggle. Two deaths due to plastic bag asphyxia are presented. Both the autopsies were performed at the University Hospital Mortuary, Kuala Lumpur. Both victims were 50-year old married Chinese males. One death was diagnosed as suicide and the other as sexual asphyxia. Sexual asphyxia is generally believed to be a problem associated exclusively with the West. Specific autopsy findings are often absent in deaths due to plastic bag asphyxia and therefore such deaths could be missed when some interested parties have altered the scene and most importantly have removed the plastic bag. A visit to the scene of death is invariably useful."

Chinese style psychoanalysis - assessment and treatment of paraphilias: Exhibitionism, frotteurism, voyeurism, and fetishism

Abstract:  "Treatment of paraphiliacs in North America has met with long-term success. As a result, the treatment, especially of exhibitionism, frotteurism (scopophilias), voyeurism, and fetishism in terms of fiscal responsibility, is estimated at over $169.00 per offender. Chinese Style Psychoanalysis has been more positive, with a very high success rate. This paper describes methods and includes case histories of the authors' work with sexual paraphiliacs. Chinese Style Psychoanalysis focuses on analyzing the patient's infantile behaviors. The patients' easily remembered childhood experiences served as evidence. Eventually, the patients achieved insight and understood that their paraphilic behaviors and impulses were molded by their childhood psychology and behaviors, and that they were using childhood amusement to meet adult sexuality. They then gave up the paraphilic behavior on their own initiative. It is proposed that utilizing these methods for the treatment of certain paraphilias would greatly enhance methods now in use in the West."

Development of the fetish interest scale: A measure of sexual interest using Forced-Choice and Visual Reaction Time Methodologies

Abstract:  "Sexual offending behavior is recognized as a major societal concern in need of further investigation. Studies of sex offender recidivism have identified paraphilic interest as a significant dynamic risk factor for future sexual offending behaviors. Although psychometrically reliable and valid assessment tools exist for a number of paraphilic disorders (e.g., pedophilia), the scientific literature describes few such measures for assessing for interest in fetish themes. This study describes the development and initial psychometric properties of a new measure of sexual interest, the Fetish Interest Scale [FIS], that uses both subjective (forced choice) and objective (visual reaction time) methods to determine the degree of sexual interest in paraphilic visual stimuli. The Fetish Interest Scale measures interest in the following seven categories: (1) adult women, (2) female feet, (3) female hair, (4) female hands, (5) panties and bras, (6) shoes, and (7) stockings. The scale was developed in three stages. During the first stage, operational definitions for each of the seven measured categories were elicited and used to identify a population of visual images that were categorized and classified during the second phase of this study. The goal of the second stage was to arrive at an empirically selected sample of images to be included in the final stage of development. Two hundred and eighty images were categorized into discrete groups and rated to help identify 126 items to be included in the third phase of development. The final phase of this study examined the psychometric properties of the Fetish Interest Scale. Participants viewed a series of single images that represent the primary content areas for the purpose of measuring reaction time (measured in milliseconds). Participants also viewed a series of images presented side by side (e.g., feet vs. hair) to determine which item they found to be most sexually interesting. Results from bivariate correlations identified a significant relationship between the visual reaction time and forced-choice preference data. Significant methodological concerns, however, temper this finding. A critical examination of the research design and suggestion for future research is discussed."
